Asociación entre sedentarismo y mortalidad según índice de masa corporal en personas mayores

Resumen

Introducción: El sedentarismo es frecuente en personas mayores y se asocia con fragilidad, dependencia funcional y mayor mortalidad. El efecto de esta asociación podría variar según el índice de masa corporal (IMC), aunque la evidencia disponible continúa siendo heterogénea en personas mayores.

Objetivo: Determinar la asociación entre sedentarismo y mortalidad en personas mayores según las categorías del IMC.

Métodos: Estudio observacional de cohorte retrospectiva basado en datos del CEMENA Frailty Study, que incluyó personas mayores atendidas en el Centro Médico Naval del Perú entre 2014 y 2020. Se excluyeron registros con datos faltantes. El sedentarismo fue evaluado mediante la escala Physical Activity Scale for the Elderly y la mortalidad se obtuvo de registros institucionales. El IMC se clasificó en cuatro categorías. Se empleó regresión de Cox para estimar el riesgo de mortalidad ajustado por variables sociodemográficas y clínicas.

Resultados: El 45,44 % de los participantes fueron clasificados como sedentarios y el 10,59 % falleció durante el seguimiento. En el análisis estratificado por IMC, el sedentarismo se asoció con mayor riesgo de mortalidad en personas con bajo peso (HR = 3,27; IC95%: 1,30–4,58) y obesidad (HR = 6,33; IC95%: 1,03–8,70).

Conclusiones: El sedentarismo se asocia con mayor riesgo de mortalidad en personas mayores, especialmente en aquellas con bajo peso u obesidad según el IMC. Estos hallazgos resaltan la importancia de intervenciones orientadas a promover la actividad física y un adecuado estado nutricional en la vejez.
