Factores asociados a recurrencia de crisis en la cirugÃa de epilepsia del lóbulo temporal farmacorresistente
Resumen
Introducción: La epilepsia del lóbulo temporal es la forma más común de epilepsia focal, un tercio de los pacientes son farmacorresistentes, la cirugÃa en estos casos tiene una relación de costo - efectividad superior al tratamiento médico.
Objetivos: Identificar si existe relación entre las variables de la fase prequirúrgica, transquirúrgica y postquirúrgica y la recurrencia de crisis (Engel II-IV), en pacientes con epilepsia temporal farmacorresistente sometidos a cirugÃa de epilepsia.
MetodologÃa: Se realizó un estudio descriptivo con una serie de 24 casos, evaluados e intervenidos quirúrgicamente por epilepsia focal del lóbulo temporal. A todos los pacientes se les realizó una historia clÃnica detallada, videotelemetrÃa, resonancia magnética y neuropsicologÃa. Se utilizaron técnicas resectivas y la electrocorticografÃa prerresección y posresección para ajustar el sitio de resección. Se evaluaron a los pacientes con la escala de Engel a un año de la cirugÃa. Se utilizó el test exacto de Fisher para determinar la asociación entre las variables. El valor de p≤ 0,05 fue considerado significativo.
Resultados: Al año de la cirugÃa, el 79,2 % de los casos estuvo libre de crisis (Engel I). La resección incompleta de la zona epileptogénica (p= 0,028), la presencia de crisis postoperatorias agudas (p= 0,036) y en los primeros 6 meses de la cirugÃa (p= 0,002), fueron factores asociados con la recurrencia de crisis (Engel II-IV).
Conclusiones: La mayorÃa de los pacientes operados estuvieron libre de crisis al año del tratamiento. La resección incompleta de la zona epileptogénica y la presencia de crisis postoperatorias agudas se relacionaron con recurrencia de crisis.
