Perforated gastroduodenal peptic ulcer sealed by the left lobe of the liver

Introduction: Perforated gastroduodenal peptic ulcer is defined as a severe condition of acute surgical abdomen, characterized by the rupture of the base of a preexisting ulcer, which causes the gastric contents to leak into the abdominal cavity.
Objective: To present an unusual clinical case of a patient with perforated gastroduodenal peptic ulcer.
Clinical case: A 62-year-old male patient with a personal history of peptic ulcer, who presented to emergency with intense abdominal pain, "stab-like," that quickly spread throughout the abdomen, associated with vomiting, general malaise peritoneal reaction, cutaneous hyperesthesia and "plank abdomen"; a perforated gastroduodenal peptic ulcer was suspected, but the abdominal X-ray did not show pneumoperitoneum. It was decided to perform surgery due to the urgency of the condition, and during the procedure it was found that the left lobe of the liver was hiding the defect.
Conclusions: Perforated gastroduodenal peptic ulcer is a rare condition nowadays, and the left lobe of the liver acting as an occlusive dressing hides the lesion, causing complications and erroneous or late diagnosis.
