Syncope as a form of presentation of Pulmonary Thromboembolism

Introduction: Pulmonary thromboembolism is a common cardiovascular emergency in intensive care units and syncope as a form of presentation is rare.
Objective: To present a patient with pulmonary thromboembolism with a high risk of mortality, with syncope as a form of presentation and favorable clinical evolution.
Clinical Case: A 50-year-old male patient with a history of having undergone surgery for a stuck epigastric hernia 15 days earlier, with favorable evolution. He reported fatigue, involuntary body movements and loss of consciousness on 2 occasions. In the complementary tests, hypoxemia, respiratory alkalosis and an S wave in DI, a negative Q and T wave in DIII were confirmed. The Doppler ultrasound of the lower limbs showed left popliteal deep vein thrombosis. Treatment with heparin and warfarin was started; the patient evolved favorably. Later, he began with sudden dyspnea, hypotension and precordial pain; It was interpreted as a pulmonary thromboembolism with a high risk of mortality and thrombolysis was performed. The clinical parameters improved.
Conclusions: Syncope is a rare presentation of pulmonary thromboembolism and the search for elements that suggest this entity is essential in its diagnosis and has an impact on the patient's evolution.
