Pseudoaneurysm of an arteriovenous fistula and two associated giant cephalic vein aneurysms

Introduction: Pseudoaneurysms at the arteriovenous fistulas and venous aneurysms are common complications in dialysis patients. Aneurysms affect all layers of the vessel wall, while pseudoaneurysms are hematomas contained by the surrounding tissue.
Objective: To describe the approach and outcome of surgical treatment of a case of pseudoaneurysm at the level of an arteriovenous fistula and two giant aneurysms of the right cephalic vein.
Clinical Case: We present the case of a patient with an aneurysmal arteriovenous fistula in her right arm. After closure of this fistula, the patient developed redness and pulsatile swelling at the surgical site. She was evaluated by vascular surgery, where a pseudoaneurysm was diagnosed in the elbow crease. Surgical removal was decided upon. During the procedure, the pseudoaneurysm was removed, and the brachial artery was repaired with a saphenous vein patch. In a second surgery, two giant aneurysms occupying the lateral aspect of the same arm were removed.
Conclusions: The removal of pseudoaneurysms at the arteriovenous fistula and giant cephalic vein aneurysms in two stages is a safe and effective strategy for resolving complications.
