I-Scan 2 virtual chromoendoscopy for the dysplasia detection in ulcerative colitis

Introduction: Colonoscopy allows for the detection of premalignant lesions and the prevention of colorectal cancer associated with ulcerative colitis. Surveillance has evolved from traditional morphological approaches to advanced imaging tools and the integration of artificial intelligence.
Objective: To analyze the clinical benefits, technical limitations and ethical-philosophical dimensions of the I-Scan 2 system in the detection of dysplasia in ulcerative colitis.
Development: The I-Scan 2 system is an advanced digital image-processing platform that modifies the light spectrum to enhance the contrast of surfaces, borders, and vascular patterns, facilitating the detection of dysplasia without the use of dyes. The use of I-Scan 2 increases the adenoma detection rate and facilitates real-time histological characterization. Its responsible adoption faces limitations such as the risk of overdiagnosis, the need for a learning curve, and inequalities in access to technology.
Conclusions: I-Scan 2 represents an advance in the detection of dysplasia in ulcerative colitis. Its adoption in Cuba must be based on evidence and conditions that ensure equitable access. Technology must serve the patient; therefore, its incorporation must be subordinate to strategies that guarantee justice and technological sovereignty.
