Tumor del estroma gastrointestinal de localización infrecuente

Resumen

Introducción: Los tumores del estroma gastrointestinal (GIST) son lesiones mesenquimales poco frecuentes, con una incidencia menor al 1 % en el colon.

Objetivo: Presentar el caso de un GIST de localización infrecuente en el colon transverso.

Caso Clínico: Paciente masculino de 62 años, con antecedentes de hipertensión arterial esencial, que acudió por cambios en el hábito intestinal y síndrome febril agudo inespecífico. El examen físico reveló un tumor palpable en hipocondrio y flanco derecho. Los estudios de imagen identificaron un tumor exofítico heterogéneo en el colon transverso y otro suprarrenal derecho, lo que sugirió una neoplasia de colon con metástasis. Durante la laparotomía, se encontró una tumoración de 15 cm, adherida a las estructuras vecinas, por lo que se realizó hemicolectomía derecha ampliada. La biopsia excisional de la lesión suprarrenal confirmó una metástasis de GIST. El estudio histopatológico e inmunohistoquímico de la pieza colónica (CD117 y DOG1 positivos) confirmó el diagnóstico de GIST.

Conclusiones: El diagnóstico de los GIST de localización colónica, con metástasis suprarrenal, requiere un alto índice de sospecha y un abordaje multidisciplinario. Este caso enfatiza la rareza de la localización colónica y la importancia de la cirugía como pilar fundamental del tratamiento.
