Surgical outcome of endoscopic endonasal approaches in pituitary adenomas over 15 years

Introduction: Pituitary adenomas are common tumors of the sellar region. Surgical resection using endoscopic endonasal approaches constitutes an effective therapeutic option; with a low rate of postoperative morbidity and adequate imaging, endocrinological and ophthalmological control.Objective: To describe the results of the use of endoscopic endonasal approaches in patients with pituitary adenomas.
Methods: A cross-sectional study was carried out that included 574 patients at the Hermanos Ameijeiras Hospital with a diagnosis of pituitary adenomas who underwent surgery using endoscopic endonasal approaches, in the period from 2009 to 2023. Variables were evaluated: sociodemographic, imaging and histopathological of the tumors, degree of surgical resection, postoperative control and complications. For data analysis, absolute and relative frequencies were used as summary measures.
Results: A 91.3% tumor resection was reported, with endocrinological control in 90.1% and ophthalmological improvement in 87.6%. In relation to the main complications, the following were identified: transient diabetes insipidus 4.0%, hypopituitarism 2.6% and cerebrospinal fluid fistula 2.1%.
Conclusions: Endoscopic endonasal approaches for pituitary adenomas are safe and effective surgical procedures with a low incidence of complications.
