Osborn wave in a patient with intraparenchymal hemorrhage

Abstract

Introduction: The Osborn wave was described in situations of hypothermia and other clinical conditions, which alter ventricular repolarization. Despite its benign nature, in generally young populations, its presence is associated with a worse prognosis.
Objective: Present a clinical case that relates the presence of a hemorrhagic stroke to the appearance of the Osborn wave.
Clinical Case: 59-year-old patient with a personal history of high blood pressure; with manifestations of loss of consciousness and muscle strength, who was diagnosed with intraparenchymal hemorrhage. During the patient hospitalization, the state of consciousness deteriorated and the electrocardiogram confirmed the presence of an Osborn wave in leads DI, DII, DIII, aVF, V2-V6, without concomitant elevation of the ST segment.
Conclusions: Elevated levels of catecholamines and myocardial damage in the context of cerebrovascular disease cause alterations in repolarization, including the J wave, and worsen the patient's prognosis.
