Introduction: Difficult intubation is defined as the need for 3 or more attempts to intubate the trachea or more than 10 minutes to achieve it. It is vitally important to detect the presence of a difficult airway in time to devise a strategy to achieve intubation of the patient. Objective: To describe the management of a patient with a difficult airway who could not be intubated and the use of a laryngeal mask was necessary for his surgical intervention.
Case presentation: A 25-year-old patient was brought to the room to undergo surgery for acute appendicitis. He had signs of a difficult airway, so the conditions were prepared to treat him, but he was not even able to intubate, it was necessary to put a laryngeal mask and continue the rest of the procedure.
Conclusions: The difficult airway, sometimes not even being prepared, can intubation be achieved, which often constitutes an enormous risk for the patient and a real challenge for the anesthesiologist, who must never face this type of situation alone, he must ask for help in a timely manner.
