Waist circumference as a predictor of ischemic heart disease in patients with type 2 diabetes mellitus

Introduction: Waist circumference is the best clinical indicator of abdominal visceral fat and is valuable for establishing a prognosis for comorbidities.
Objective: To determine the cutoff point for waist circumference as a predictor of ischemic heart disease in patients with type 2 diabetes mellitus.
Methods: A case-control analytical study was conducted in patients with type 2 diabetes mellitus treated at the Bayamo Diabetes Care Center, Granma province, Cuba, from January 1, 2015, to December 31, 2020. Sensitivity, specificity, and positive and negative predictive values were calculated for each waist circumference cutoff point.
Results: The optimal waist circumference cutoff point for men was 100 cm with a sensitivity of 75.0%, a specificity of 75.6%, a positive predictive value of 56.25%, and a negative predictive value of 87.8% (area under the ROC curve = 0.753; 95% confidence interval = 0.684–0.822; p = 0.000). For women, the cutoff point was 99 cm with a sensitivity of 56.9%, a specificity of 77.4%, a positive predictive value of 64.4%, and a negative predictive value of 71.4% (area under the ROC curve = 0.672; 95% confidence interval = 0.602–0.742; p = 0.000).
Conclusions: A waist circumference of 100 cm in men and 99 cm in women is a predictor of ischemic heart disease in patients with type 2 diabetes mellitus.
